Travel Document

A Philippine citizen who, for one reason or another, is unable to have a passport to return to the Philippines, may be issued a Travel Document valid for one direct flight to the Philippines. Please contact the Philippine Consular Office in Auckland.

Notarials - Acknowledgement

Personal appearance is required for the acknowledgement of the following documents: Special Power of Attorney, Affidavits, Parental Support & Consent, Certification of Documents and Oath Taking.

Please bring your passport. Documents must be signed and dated in front of a consular staff in the Auckland office.

The below forms have been prepared in Microsoft Word Doc (.doc) format. We advise that you download the necessary document, type in your details and bring a printed unsigned copy with you. Please note, the document should be dated the same day you visit the consular office.

Notarials - Authentication

In the event that the person whose signature must appear in the document is unable to appear in person, the following procedure is necessary:

  • The legal document should be notarized by a New Zealand Notary Public. Fee charged by a Notary Public can range from NZ$30.00 and up.
  • The duly notarized document should then be submitted/posted to the Authentications Office of the New Zealand Department of Internal Affairs in Wellington (04-4702928), which will issue a certification on the notarial seal and signature of the Notary Public who certified the primary document. Payment of NZ$80.00 (to cover the Department of Internal Affairs charges and the Ministry of Foreign Affairs charges for the following procedure) may be made by cheque or money order made out to the Department of Internal Affairs or by providing Visa/MasterCard details.
  • The Certification of the Department of Internal Affairs, together with the primary document, shall thereafter be forwarded to the Consular Division of the New Zealand Ministry of Foreign Affairs and Trade (MFAT) in Wellington. This Office will further certify the certification issued by the Department of Internal Affairs.
  • After the NZ MFAT has authenticated the document, this should be forwarded to the Philippine Consular Office in Auckland for final authentication. Consular service fees apply and a pre-paid courier bag for the return of your documents is required. Please contact the Philippine Consular Office in Auckland prior to sending your documents.

Notarials - NBI Clearance Form

For NBI Clearance, the consular office performs both the fingerprinting and acknowledgement of the personal appearance. Please bring one (1) photo ID and your passport. The completed and stamped NBI Clearance form can then be sent to any NBI office in the Philippines for further processing.
